EP RATTLERS – COLLEGE PRACTICE TRIP CONSENT
Participation Agreement & Release
1. Participation Acknowledgment
I understand that my child is voluntarily participating in an EP Rattlers practice trip to a college campus. Activities may include transportation to and from the campus, wrestling practice, drills, live wrestling, conditioning, facility tours, and other activities directed by the host coaching staff. I understand that wrestling is a contact sport and involves inherent risks, including but not limited to sprains, strains, fractures, concussions, illness, or other injuries.
2. Assumption of Risk
I knowingly and voluntarily assume all risks, both known and unknown, associated with my child’s participation in this trip, including transportation to and from the college campus and participation in all wrestling-related activities.
3. Transportation Authorization
I authorize my child to travel to and from the designated college campus with EP Rattlers coaches, staff, volunteers, or other approved drivers. I understand reasonable efforts will be made to provide safe transportation, but I acknowledge that travel carries inherent risks.
4. Medical Authorization
In the event of an injury or medical emergency, I authorize EP Rattlers coaches, staff, or representatives to obtain emergency medical treatment for my child. I understand that I am responsible for any medical expenses incurred.
5. Health Information
I certify that my child is physically able to participate in wrestling activities. I have disclosed any known medical conditions, allergies, medications, or special needs that may affect participation.
6. Conduct Expectations
I understand that my child is expected to represent EP Rattlers in a respectful and sportsmanlike manner while traveling and while on the college campus. Failure to follow the instructions of coaches or host staff may result in dismissal from the activity and arrangements for early pickup if necessary.
7. Release of Liability
I release, waive, and discharge EP Rattlers Wrestling Club, its coaches, volunteers, staff, approved drivers, host institutions, facility providers, and affiliates from any and all claims, liabilities, demands, or causes of action arising from participation in this trip, except in cases of gross negligence or willful misconduct.
8. Photo & Media Release
I grant permission for EP Rattlers Wrestling Club to use photographs and video taken during the trip for promotional, educational, and social media purposes.

By checking this box, I acknowledge that I have read, understand, and voluntarily agree to the terms of this Participation Agreement & Release.
